Monthly accountability sessions
- Follow up sessions allow us to track your progress, and support you in building long-term healthy lifestyle habits. We’ll focus on addressing challenges and developing strategies to keep you on track with your action plan.
- Discuss your personalized wellness action plan with you, review results from your testing, discuss your progress, and work with you to adjust your wellness action plan to optimize your wellbeing.

There are some costs that are not included in your membership and may require out-of-pocket expenses:
- Lab tests. Many tests may be covered partially or fully with insurance, but some may require a copay or may not be covered at all.
- Complementary healing modalities and other health care services recommended by Carter Hall Lifestyle, such as annual physicals, psychotherapy, acupuncture, medications (both prescription and over-the-counter), and supplements.
We applaud you for taking the first step towards achieving lifestyle balance. Keep in mind that a positive lifestyle is not just about your physical health – it also incorporates your mental and emotional health. While habitual positive lifestyle behaviors may take some time to develop, they are well worth the effort as your quality of life is largely dependent on your health.
We want to support you in committing to positive lifestyle behavior changes and to do so we are asking for a 12-month commitment. Doing something for the first time takes preparation, intention, consistency and patience. It’s not about willpower. Positive habits happen when we set ourselves up for success. The Collective will provide you the foundational framework to incorporate these positive habits in your daily routine. Allow yourself the year to cultivate these healthy habits and strengthen the mindset to sustain them in the long-term.
Carter Hall Lifestyle offers tiered membership options based on the number counseling sessions with a registered dietitian you receive in a year. Those membership options include monthly, bi-monthly or quarterly visits. Your membership price is based on the number of visits you’d like to have with a dietitian over the course of your annual membership.
Additionally, Carter Hall Lifestyle offers savings for annual memberships paid in full at time of enrollment. We offer a 7% discount for upfront annual membership payments.
You’ll work with a registered dietitian to dive deeper into your health history to identify the root cause of your symptoms. Your dietitian likely will contact your primary care physician (PCP) to discuss any underlying conditions you may have. Together we’ll work to develop a health plan for long-term care focused on healing once your dietitian has a clear picture of your medical history and current lifestyle behaviors.
Here is what a year with The Collective looks like:
GETTING STARTED
- Complete in-depth medical intake form which will provide insight into your overall health.
- Schedule a 60-minute initial appointment with a dietitian to discuss your complete medical history and current lifestyle behaviors (nutrition, fitness and wellness).
ASSESSMENT + INSIGHTS
- Get your personalized, comprehensive action plan to address any underlying conditions with appropriate medical treatment, including recommendations for better nutrition, sleep, stress management, healthy movement, incorporating supplements and medication as needed.
- Complete dietitian recommended blood work and advanced specialty testing tailored to your health goals. Review results with your dietitian and receive recommendations to treat any acute or chronic conditions (e.g gut dysbiosis, hormonal imbalances, food sensitivities, systemic inflammation), manage your symptoms and heal [premium memberships].
TREATMENT + PROGRESS
- Begin your personalized health plan, including nutrition, stress reduction, exercise + sleep optimization, supplements, and medication, if necessary.
- Utilize unlimited messaging via the Kareo EHR portal to get answers to any questions you may have throughout the process.
- Reassess your plan and conduct additional testing as needed.
SUPPORT + RESULTS
- Track your progress with monthly accountability sessions and continue optimizing your health with the help of your squad.
The Collective membership is designed for adults, 18 years of age or older, who are looking to commit to healthy lifestyle behavior changes. This membership is not ideal for someone who is looking to quickly meet weight loss goals through dieting and restriction. The Collective framework follows an anti-diet and holistic approach, teaching its members how to rely on functional nutrition, daily physical activity and prioritized self-care.
The Collective membership aims to help you find lifestyle balance with the support of healthy lifestyle behaviors. After completing one year in the membership, you should be able to plan meals and program regular physical activity to support metabolic flexibility and prioritize self-care and regeneration. Together, these aspects provide the framework for long-term lifestyle balance.
Billing & Insurance

When signing up for The Collective membership, you’ll have the option to either pay monthly or annually, at which time you’ll need to provide your credit card information.
If you choose to pay monthly, you’ll be charged the first monthly payment on the date you sign up, and every month following the initial payment. If you choose an annual payment, you’ll be charged the total amount upon signing up.
Keep in mind – monthly membership fees are designed to cover all of the services and products The Collective provides and not solely your monthly visits with the dietitian.

The Collective Guarantee
On average it takes 2 months- 66 days to be exact- before a new behavior becomes habitual and automatic. How long it takes an individual to form a new habit can vary widely depending on the behavior, the person, and the circumstances. In fact, some studies say it can take anywhere from 18-254 days to build a new behavior into your life!
Here’s our guarantee to you… If after three months, you’ve found that you are not finding alignment with our support, then you are free to leave the membership*.
Your membership is set to terminate at the one year mark. Ideally, the goal is that you participate in the membership for the entire year, then take what you’ve learned and move on. You should be able to sustain the positive lifestyle behavior changes made throughout the year, but just in case you still require support, you are welcome to stay in the membership longer. If this is the case, be sure to reach out to a membership advisor one month before your membership is set to terminate and we’ll discuss your renewal options.
Your insurance may cover your sessions with the registered dietitian nutritionist. Eligible Collective members can use their insurance for their visits and pay a lower annual membership fee. Ms. Carter is a preferred provider for Blue Shield of California, Anthem Blue Cross of California and United Healthcare.

Prior to joining, we recommend you call the member services phone number on the back of your insurance card to learn more about your potential out-of-network coverage. Be sure to ask what your specific out of network benefits are and what your potential reimbursement may be.
After each session, we will prepare a medical invoice or “superbill” that details the information your insurance company needs to process your claim, including CPT codes for services performed, as well as ICD-10 codes for diagnoses. Afterward, you’ll need to submit the superbill to your insurance company and follow their specific claim process.
We know this can be a lot, so if you have any questions while going through the process, you can always reach out to a membership adviser for support.
Can I use FSA or HSA to cover membership or other costs?
We recommend checking with your FSA/HSA company for more information and details as to how you can use these funds. We will be more than happy to provide you with the appropriate documentation if requested.
Lab Testing
Many of our health concerns require more diagnostic testing to determine the underlying causes. Through Rupa Health, our dietitians have the ability to order specialty tests for allergies, cortisol, genetics, microbiome health, and toxic metal levels, and may also include saliva, urine, and stool testing.
The majority of specialty testing is billed directly to you as an individual rather than through insurance. However, many of these tests can be submitted for reimbursement or paid for using FSA/HSA funds. Since this testing does not typically go through your insurance, we can provide upfront costs and guide you on how to submit for reimbursement. Specialized tests are optional and not typically covered by insurance. Cost generally ranges between $100-$400.
If these costs are not financially available to you, don’t despair. Our dietitians will work with you to customize your wellness action plan based on your needs. Our recommendations are based on what we feel will help you improve your health and wellness and #LiveNourished; we are not incentivized to order these tests in any way.

Virtual + In-Person Visits
Currently all individual and small-group sessions with the dietitian as well as educational webinars will be held virtually. Physical sessions are pending for our flagship office in Los Angeles, California Fall 2023. You will receive the same quality of care whether we meet virtually or in-person.

Credentialed nutrition and dietetics practitioners providing telehealth services where the practitioner and patient are located in different states, the practitioner providing the patient care service must be licensed and/or meet the other applicable standards that are required by state or local laws in both the state where the practitioner is located and the state where the patient is located. Note some states have exemptions that may enable one to practice even without being licensed in that state.
If you currently reside outside of California, please give us a call to verify we are licensed in the state in which you reside.
At least two days before your visit, please confirm your visit date, time, and location details in the Kareo electronic patient portal. Prior to your initial session, be sure to complete the pre-session paperwork including: Intake/Lifestyle Questionnaire, Consent for Telehealth, Notice of Privacy Practices.
You will be provided with a GoToMeeting virtual session link 48 hours prior to your visit. Please check the patient portal and your email for this link.
You’ll receive reminder emails with your visit details, so be sure to read them thoroughly. Our office is available to answer any questions you may have or reschedule your appointment if needed.
When your initial session is scheduled, you will receive the details of your appointment via email. Included in that email are links to electronically sign the Consent for Telehealth and Notice of Privacy Practices documents.
Your intake form/lifestyle questionnaire is provided to you as a filled PDF form. Please complete the document in full and submit it via the Kareo Patient Portal. To do this, simply create a message and attach the intake form to the message.
Filling out your pre-session paperwork prior to your session allows us to make the most out of your visit. That’s why we ask that all questionnaires be filled out 48 hours prior to your initial visit. If you need more time to complete yours, we would be happy to help reschedule your visit for your convenience.
